Request the HHM Health Mobile Unit

Bring pediatric healthcare directly to the children and families in your community. The HHM Health Pediatric Mobile Unit partners with schools, faith-based organizations, community groups, employers, municipalities, and other organizations to improve access to preventive and primary care services for children from newborn through 18 years of age.

To help us safely plan staffing, supplies, clinical services, and scheduling, all mobile event requests must be submitted at least 30 days before the preferred event date. Please submit the form below with complete and accurate information. A member of the HHM Health team will review the request and respond within five business days.

Pediatric Services

Organizations may request pediatric clinical services for children from newborn through age 18. Available services may vary by event and are subject to clinical review, staffing, supplies, eligibility, and HHM Health policies. Requested services may include:

• Pediatric examinations and preventive visits
• School and sports physicals
• Developmental, vision, hearing, and age-appropriate health screenings
• Immunizations when approved, available, and clinically appropriate
• Acute or minor illness evaluation when included in the approved event scope
• Health education for parents, guardians, children, and adolescents
• Referral and follow-up coordination with an HHM Health clinic
