If you are currently enrolled as a patient with HHM Health, please go to our Patient Portal in order to schedule an appointment, contact your provider, and receive more information.
Providing accessibility to everyone, uninsured or underinsured, HHM Health accepts a wide variety of insurance including Medicaid, Medicare and private insurance.

Enrollment

HHM Health is  a Federally Qualified Health Center. Patients are screened based on income to determine eligibility for a sliding fee discount.

How To Enroll & Your First Visit

Step 1:

Visit the HHM Health office nearest to you with the required documents listed below and receive your enrollment documents between the hours of 8:00am – 4:00pm.

Step 2:

Complete the enrollment documents in our office. Assistance is readily available, including multiple translators.

Required Documents

Please make sure to bring all required documents at time of registration.

Valid photo ID.
Most recent pay stub of everyone in the household who works plus any other type of income, such as SNAP Food Benefits, child support, disability, Social Security, etc.
Certified Disability letter, Section 8 Housing documentation and/or SNAP Food Benefits Letter.
Completed enrollment forms.  Pick up forms at our clinic, or download, print and complete here.

HHM Health provides quality care. Our services are not free, but come with great value.  Our patients are eligible for a sliding fee discount based on the Federal Poverty Guidelines based on income and family size.  We request payment from patients at the time of service in the form of a co-pay for those insurance plans that require it.  Patients with charges going toward deductibles and coinsurance will be responsible for those charges at the time of service.
